Two (2) positions available and are located geographically for Anderson County and/or upstate; one (1) position for Berkeley County.

Under general supervision, identify and execute activities that result in patient discharge from inpatient settings to integrated community residences and community based behavioral health services, to include discharge plan development, accessing residential settings, patient assessment and linking to commnity based providers.

Conduct initial clinical assessments and plan of care with patients choosing community mental health center for outpatient provider to support continuity of services from inpatient settings. Coordinate and locate community based residential settings options, based on patients' discharge plan.

Provide face-to-face and/or video introduction of patient to community providers to include care coordinator and peer support specialist.

Attend social worker meetings and treatment teams in the hospitals to provide community based resources and/or options for discharge planning.
Identify, collect and routinely update inventory of integrated community based residences for Transition Team distribution/resource directory. Collect data for performance improvement.

MINIMUM AND ADDITIONAL REQUIREMENTS:
Agency Requirements: A master's degree in guidance, counseling, or the social sciences and one (1) year of experience in a clinical counseling program.

Note: Must possess ability to access and coordinate interaction between multi-disciplinary staff and have extensive knowledge of psychopathology and patient management within a psychiatric setting.

Starting Salary Range: $33,494.00 to $46,799.00 /year
