MEDICAL RECORDS

  • Medical records are not public documents and are therefore not subject to the Freedom of Information Act. If you are seeking copies of your treatment records, please follow the instructions on our Medical Records Request page, here: https://scdmh.net/patients_families/how-to-request-medical-records/. This page provides the form you will need and the addresses and phone numbers of all SCDMH facilities.

EMPLOYMENT RECORDS

  • If you are an employee or former employee seeking a copy of your employment records, a Freedom of Information Act is not required. Please contact SCDMH Human Resource Services at (803) 898-8607 to make a request.
  • If you are a member of the public seeking employee information, a Freedom of Information Act request is required. Please note that not all employee information is public information and SCDMH will make redactions in accordance with South Carolina’s Freedom of Information Act statute, section 30-4-40.
